What services do you provide?
IUD counseling and insertion for clients, any parity (first midwifery led IUD clinic in Ontario)

Nutrition, prenatal, and new mother group classes
Newborn care and postpartum homevisits (in partnership with Hamilton safe transitions group for clients in wards 3 and 4 who currently do not have a family doctor on discharge from the hospital)

Who do you provide services to?
We are prioritizing care to clients within Hamilton Wards 3 and 4, with a goal of improved access to care for these individuals. Clients do not have to be patients at Crown Point Family Health Centre to access the midwifery services.
Who is part of the team?
We are happy to be working in a multidisciplinary team. Our midwifery core team includes Mona Abdel-Fattah, Kate Demers, Chelsea Futers and Christine Sandor. We are integrated within the Crown Point Family Health Centre and therefore also work with the on-site family doctors, dietitians, nurses, pharmacist, mental health counselors and administrative staff. Dr. Dustin Costescu is our gynecologist lead. Dr. Meghan Davis is the project’s family doctor lead. Crown Point is also part of the Hamilton Family Health Team.
How do you reduce barriers to accessing care?
We offer care in Arabic, English and French. We have partnered with nearby services including pharmacy and ultrasound clinics that are within walking distance of our location and we are near major bus routes to minimize transportation barriers. We offer our services with minimal wait times, from next day to within 2 weeks depending on the service and urgency of the visit. On-site clinic visits also boast free parking.
How do patients access your services?
Clients can self-refer by calling the Crown Point Family Health Centre at 905-547-2302. We are also happy to accept referrals from family doctors. You will find our referral form in this package
Is there any negation for rostered patients?
No. All patients accessing care will only be seen by one of the Crown Point Midwives so there is no negation involved. Primary care providers working in a patient enrolled model of care do not need to de-roster enrolled patients seen by Crown Point Midwifery.
When did you start?
Our proposal for an Expanded Midwifery Care Model was accepted by the Ministry of Health and Long Term Care in early 2018. We spent the first few months planning services and officially started seeing clients in September 2018. Since we opened, we have provided thousands of client appointments across all of our services.
